As a courtesy to patients, we will contact your insurance company for verification of benefits, however, it is the patient’s responsibility to:
- Know their insurance policy. Patients should be aware of their benefit coverage, covered and non-covered services, authorization requirements, all cost share information such as deductibles, coinsurance, and co-payments.
- To obtain a referral from their Primary Care Physician or Specialist and/or obtain authorization for treatment from their insurance company prior to receiving services.

DIRECT ACCESS: The State of California allows for direct access, California Law AB 1000, enabling a patient to seek physical therapy care without a physician referral. Direct access enables a patient to receive treatment for 12 visits within a 45 day time frame. Many insurance companies acknowledge direct access, however it is important for you to check with your insurance if a physical therapy prescription is required for insurance coverage.
